SNaP's Medical Emergency Card features a section for personal information (name, address, phone number, date of birth, allergies to meds, as well as optional information) and a section for contacts and phone numbers (family and friends, physicians, insurance information, etc.). The reverse side of the card allows you to list your medications, the dosage, and times per day you take the meds. Keep several copies of this card and update it as necessary. Keep this card on your refrigerator, in your wallet and car and with your children.
The card is presented here as a PDF document of two pages. The simplest way to deal with it is to print the document (both pages), then flip the two pages over, end-for-end, and feed them into your printer to reprint the document on the backs of the two pages. That way you should get two copies of the card, each printed on front and back. Or, if you like, just call Donna at 352-2612 and she'll get a couple of the cards to you.
