The membership application form is presented in two files, which comprise the front and back of a single sheet. If you would like to print the form so that you can fill it out, we ask that you print one page, then turn the paper over and print the second page on the other side of the same sheet. You can either mail or hand-deliver the completed form to our Executive Director at 3872 Alfalfa Lane, Fayetteville, PA 17222. |
